What is Dermal Filler?
Dermal fillers are injectable treatments which aim to soften the appearance of fine lines and wrinkles, as well as restoring definition and volume loss of the face.
Injectable Hyaluronic Acid (HA) is present in most types of temporary dermal fillers. Once injected into our skin, HA attracts water, which helps to restore volume, recreate lost structure, and replenish our bodies’ natural sources of HA. This, in turn, reduces the overall sunken or sagging appearance of the face, revitalising and adding volume to the skin and softening the overall look of fine lines and wrinkles.
Dermal fillers are non-permanent because the body usually breaks them down over a period of time.

From the 1st October 2021, The Botulinum Toxin and Cosmetic Fillers (children) Act 2021 was introduced, making it a criminal offence to administer Botulinum Toxin (hereafter referred to as Botox) or a Filler by way of injection for a cosmetic purpose, to a person under the age of 18. It is also an offence to make arrangements to book an appointment to provide these treatments to anymore under the age of 18.

Types of Dermal Filler
Dermal fillers differ in chemical makeup, and longevity and have varying degrees of softness (the G prime). Softer fillers are used in the lips, for example, while denser fillers might be desired to enhance cheekbones.

Dermal fillers for Lip Augmentation
Lip augmentation can be achieved when dermal fillers are injected into the lips to create volume and shape.
Dermal fillers for the lips have a softer G prime (an indication of the viscosity and elasticity of the filler), which helps to mimic the natural anatomical structure of the lip. Dermal filler is made up of Hyaluronic Acid (HA), which is a natural substance found within the lips. Using either a needle or cannula, HA is injected to create shape, contour and restore volume. The amount of dermal filler used and the final look will be tailored to the client.
Lip fillers can be used to reduce signs of ageing and smokers’ lines too.
HA breaks down naturally within the body and generally lasts up to 6-8 months.
To make this procedure comfortable, aesthetic practitioners will use a topical anaesthetic 30 minutes before the procedure. The lips will naturally bruise and swell post-procedure which can take up to 10 days to settle. The aesthetic practitioner will make the appropriate recommendations and aftercare advice.
Dermal fillers for Cheeks and Mid-Face
Dermal fillers can be injected into the cheeks to create a sense of volume (fullness) and improve the definition of the cheekbones and facial symmetry.
This can have a secondary effect of lifting the eye area and reducing the depth of the nasolabial folds as the face is gently ‘lifted’.
Fillers in the mid-face can add definition and restore fullness to the face, lifting your cheeks to reduce the appearance of sagging.

Dermal fillers for Chin, Jaw and Lower-Face
Dermal fillers can be used in the chin, jaw and lower face to help to restore fullness in areas that have lost volume with age or to create a more symmetrical or balanced profile. As we age, our chin can lose volume and recede into the jawline, which can compromise the balance and profile of the face.
Dermal fillers can be injected into the jawline and chin to create a more balanced profile. They can be administered to re-sculpt this area to rebalance your profile, giving you a more youthful, symmetrical look.
Dermal filler can also be used in the lower face to address signs of ageing, for example, to restore lost volume or to reduce the appearance of lines and wrinkles (like our nasolabial folds and marionette lines) to create a defined jawline.
Dermal fillers for the chin and jaw usually contain Hyaluronic Acid (HA) gel. The gel is injected into the area to create instant volume and definition and is often combined with chin augmentation. HA is found naturally in the body and so allergic reactions are rare. The gel can be administered in stages to create volume and definition. This allows your practitioner to create a natural, more balanced effect.
The results last up to 12 months and will be broken down naturally into the body, so additional treatments may be required from 8-12 months.
How Dermal Filler Treatment Works
Dermal filler works in two ways. Firstly, the gel is injected into the area to create volume or definition. Your practitioner can adjust the volume by adding more filler and sculpting the filler to get the look you desire.
The Hyaluronic Acid (HA) in the filler also boosts volume by attracting water, which helps to replenish the moisture in your skin. This leaves the skin looking plump and fuller, with a hydrated glow.
Preparing for Dermal Filler Treatment
You may not be suitable for dermal fillers if you:
- Have a skin infection such as a rash, blemish or cold sore
- Are unwell
- Are pregnant or breastfeeding
- Have previous nodules which may need dissolving
- Have dermatological conditions such as rosacea
- Recent infections or dental procedures within the last 2 weeks
- Are travelling abroad within 2 weeks.
Our clinician will ask you about your medical history and any medication you are taking to ensure you are suitable for treatment.
Ahead of the procedure, we ask that you:
- Do not drink alcohol or excessive caffeine
- Do not take over-the-counter drugs such as aspirin or ibuprofen which
- may thin the blood and increase your risk of bruising
- Do not wax or bleach the treatment area
- Avoid excessive exposure to the sun.
It is a good idea to schedule your appointment two weeks prior to any big event or travel. This will also allow any bruising around the area to fade.
You are advised to avoid any UV exposure for 2 weeks after treatment. Prolonged UV exposure can cause your fillers to break down so that they don’t last as long. UV exposure can also slow down the healing process. Please ensure to wear broad-spectrum sunscreen after your dermal filler procedure.
What to expect during your Dermal Filler treatment
Before you have dermal fillers, you should have a thorough consultation with your practitioner. This will allow you to discuss the results you want and the realistic results you can expect to achieve. A skilled practitioner will understand the anatomy of the face and address any concerns you might have.
Ahead of the treatment, a topical anaesthetic (numbing cream) may be applied; this is usually only in the case of lip fillers. Please advise your practitioner if you would like to use this for other treatment areas.
Do not take any anti-inflammatories or blood thinners (eg ibuprofen or aspirin) as pain relief prior to your consultation. This could increase the risk of bruising.
Your practitioner will assess the area before administering the filler via a needle into the skin or cannula. You can expect some discomfort, particularly in sensitive areas like the lips, or in areas close to the bone, like the chin.
This discomfort should be mild and will resolve quickly. The procedure will usually take around 30-60 minutes to complete and your practitioner will monitor you after the treatment in case of dizziness or an allergic reaction (which is rare). We will always massage out any lumps or bumps with antiseptic cream post-treatment.
Dermal Filler Aftercare
You can take paracetamol if advised by your practitioner and it is safe for you to do so.
To avoid the small risk of infection, the skin should be kept clean and clear after a dermal filler treatment. Do not wear makeup for 24 hours post-procedure.
Do not rub or massage the skin. Try not to put any pressure on your face; for example, if you go to bed, try not to lie on your face for at least 48 hours after the treatment.
- Avoiding touching or rubbing the area to minimise the risk of infection
- Do not exercise or drink alcohol for 24 hours
- Take paracetamol if in pain (and is safe to do so) and avoid ibuprofen as this may increase the risk of bleeding
- Watch out for signs of infection and occlusion
- Avoid UV exposure and apply SPF 50 to the face
It is usual to have some redness, tenderness and swelling around the injection sites, but this should dissipate after a few hours and disappear in 24 hours. Bruising around the injection site is more common. You can apply ice and may be advised to take arnica. You can expect bruising to fade within 2 weeks.
Dermal Filler Risks & Side Effects
Having dermal fillers is usually safe when performed by an experienced practitioner. They will understand the anatomy of the face and will be able to check your medical history to ensure you are safe to have fillers.
Following on from your consultation, your practitioner will explain the risks and possible side effects and you will be required to sign a consent form.
Serious complications can occur. An experienced, fully certified practitioner will always try to minimise risks associated with various techniques and by taking an in-depth medical history.
Side Effects
The most common side effects from dermal fillers are:
- Bruising and bleeding
- Swelling and tenderness
- Post-procedure swelling, redness and tenderness
- Lumps under the skin which may be palpable
- Asymmetry, lumps or over-correction of the area.
Common Risks
The most common risk of dermal fillers is bleeding and bruising around the injection site. Bleeding should stop after the procedure. You can apply ice and may be advised to take arnica. You can expect bruising to fade within 2 weeks.
There is a small risk of infection; this should be limited by a skilled professional who will be trained in administering the treatment to minimise the risk. Keeping the area clean and free of makeup after treatment will help to reduce this risk further.
There may be some asymmetry in the face after the procedure, which can usually be corrected at 2 weeks.
Lumps under the skin or overcorrection of wrinkles (puffiness) may also occurr.
Less Common Risks
Skin damage during the procedure can lead to an infected wound which may scar. Your practitioner will advise you on aftercare and can support you if you are concerned by a possible infection.
Rare Risks
In very rare cases, filler can cause vision problems and blindness.
Severe pain, blanching and discolouration to the area are signs of a possible VO. If caught early, the VO can be reversed by your practitioner. A VO is a medical emergency and you should reach out to your practitioner who will have emergency protocols to put in place.
Removal of Dermal Fillers
Dermal fillers can be dissolved. Your practitioner may not be comfortable in removing filler that has been done at another clinic in cases where they are unable to verify the original product used. Some fillers may not be responsive to Hyalase and this can cause further complications.
Fillers should only be administered by a qualified and trusted practitioner.

If you are nervous about what the results of dermal fillers will look like, your practitioner can use a small amount in your first consultation, and can build the results over time.
